Enrolling for Veterans Administration Benefits

For general information on how a veteran in the community can enroll for benefits from the Veterans Administration visit Veterans Benefits Administration site at http://www.vba.va.gov/VBA/.

For new enrollment, complete form 1010EZ (found at web address below) and present to the eligibility office: https://www.1010ez.med.va.gov/sec/vha/1010ez/.

To Expedite Enrollment at the VA:

  1. If they have been at another VA in the country, the VA can pull the DD214 from the system and the patient has to update their financial information
  2. The Veteran appears in person with the 1010EZ form (above link) and the DD214. With these forms completed, veteran can be enrolled on site.
  3. Form 1010EZ and the DD214 can be faxed or mailed to the area eligibility office. Fax and mailed forms may take up to 2 weeks to process. St. Petersburg Bay Pines eligibility office Fax #727-319-1229.
  4. If the veteran is unable to visit the eligibility office. The hospice social worker or a family member can call the eligibility office. (Bay Pines VA phone 727- 398-6661 x.5684) let them know that the veteran who is applying is a hospice patient and ask for assistance in expediting the request.You will need to fax the form 1010EZ and DD214 and they will process right away. Or, the social worker or family member can bring the completed forms to the eligibility office and they can process right away.
